BEST MEDICINES COALITION (BMC): PRIVACY POLICY

At Best Medicines Coalition (BMC), it is our policy to respect your privacy and BMC has prepared this Policy to inform you of our policies and practices concerning the collection, use, retention and disclosure of personal information.

This policy governs personal information collected from and about:

(i) individuals/corporations who are or may become clients of BMC; and

(ii) individuals, corporations or organizations with whom BMC works with.

Using contractual or other arrangements, BMC shall ensure that agents, contractors or third party service providers and employees, who may receive personal information in the course of providing services to BMC as a part of the provision of marketing services and event planning, will protect that personal information in a manner consistent with the principles articulated in this Policy.

This Policy does not cover aggregated data from which the identity of an individual/corporation cannot be determined. BMC retains the right to use aggregated data in any way that it determines appropriate.

Personal Information To Be Collected

Personal information is defined as data that can be utilized to identify an individual/corporation. That information for an individual will include but is not restricted to, your name, address, health card number, date of birth, social insurance number, BMC reference number, and any financial data including credit card numbers that you supply. The information required will be specific to and used only for the identified purposes provided as part of the Policy of BMC

Collecting and Using Information

- BMC will collect personal information only as required for the provision of marketing services and event planning and related activities carried out by BMC.

- BMC will obtain your verbal or written consent before collecting information from a third party such as a bank or credit card company. We will protect the personal information that we gather and use it as indicated.

- BMC only collects, uses and discloses Personal Information for the following purposes:

- (i) determining and facilitating your payment of fees for services rendered related to the activities conducted by BMC

- (ii) verifying your identity and the accuracy of your personal information ,

- (iii) establishing and maintaining communications with you

- (iv) complying with the law

- (v) notifying you of certain products or services offered by us.

Sharing Information

- BMC will treat all information with great respect and will not sell personal information without your informed, written consent.

- BMC may share your personal information with BMC owners/ staff and appointed accounting, legal, and tax advisors as a matter of regular business practice but only with strict confidentiality agreements in place.

- Provide information in those circumstances where we are required to do so by the federal or provincial government, and by other law.

- In any other circumstances, we will share your information only when we have your written consent and will do so only with the intent of protecting you and your financial information, or to enhance products and services for the purpose of marketing and event planning.

Consent for the use of Information

By signing below you consent to our use of your personal information gathered to provide our services to you. We may use information to establish and service you as our client. We rely on the following actions by you as indications of your consent to our existing and future personal information practices:

- your voluntary provision of personal information directly to BMC or through a bank or other financial institution for the provision of marketing services and event planning and other related activities, products, services and programmes

- your express consent or acknowledgment contained within a written, verbal or electronic application process

- your consent solicited by BMC for a specified purpose

- your receipt of this privacy policy, until you notify BMC in writing of a withdrawal of this consent

- your consent given through your authorized representatives such as a legal guardian, parent, or holder of a power of attorney

Subject to certain legal or contractual restrictions and reasonable notice, you may withdraw this consent at any time. BMC will inform you of the consequences of withdrawing your consent. In some cases, refusing to provide either personal information or withdrawing your consent for BMC to collect, use or disclose your personal information could mean that we can not provide you with the services as set out in this Policy and/or it may affect your ability to participate in our programmes.

STORAGE

BMC maintains all personal records of its clients in locked filing cabinets which are accessible to staff on a need to know basis.

By signing below you consent to our use of your personal information gathered to provide our services to you. We will be pleased to explain your options and discuss the consequences of withdrawing your consent.

Protecting Information – Retention and Security

BMC will protect your information with appropriate safeguards and security measures. We utilize computer systems that restrict access to personal information. Our physical premises have security safeguards that assist in the prevention of unauthorized access to our facility. BMC restricts access to personal information to those employees and contractors that the management of BMC has determined need to know that information in order that BMC may provide services to its clients. We will retain your information only for the time frame that is required for the purposes as explained to you or by other law. We have established procedural requirements and trained our staff to be respectful and to comply with this Policy.

Reviewing Your Personal Information - Accuracy


BMC endeavors to ensure that any personal information provided by clients and in its possession is as accurate, current and complete as necessary for the purposes for which BMC uses that data. Information contained in files that have been closed is not actively updated or maintained.

We will allow you to review your information at any time. To guard against fraudulent requests for access, BMC will require sufficient information to allow it to confirm the identity of the person making the request before granting access or making corrections. We ask you to help us to make every reasonable effort to ensure that we keep your information accurate and current. This statement is in effect as of June 28, 2005. BMC., will from time to time review and revise its privacy practices and this Policy. In the event of any amendment, an appropriate notice will be communicated to clients and others in an appropriate manner.
